Membership Application, 2011


Membership fee for 2011 is $65.00. If you have not done so already, please review the "instructions page" for instructions on how to pay your membership dues. Your application will not be considered complete and you will not be permitted to take the annual rules test until your dues are paid.

Your membership fee includes payment for one copy of the 2011-2012 "high school" (NISCA) water polo rule book. The rule book will be mailed to you as soon as it's available from NFHS. The books will be ordered on or about July 4, 2011. Thus, to insure timely delivery of your rule book, your application for membership should be completed no later than July 4, 2011.
